INSURANCE BENEFITS
Our comprehensive and competitive insurance benefits program is designed to help employees achieve financial security and live healthier lives.

Medical Insurance

The Citizens Bank health plan is designed to provide employees and their eligible dependents with financial protection against the high costs associated with health care. The plan provides comprehensive coverage for full-time employees and covered dependents, including prescription drug coverage. The Citizens Bank covers 100% of the cost for employee coverage.

Dental Insurance

The group dental plan provides full-time employees and covered dependents a comprehensive dental protection plan, including diagnostic and preventative services, along with coverage for basic and major dental benefits. The Citizens Bank covers 100% of the cost for employee coverage.

Vision Insurance

The Citizens Bank offers full-time employees two options for vision coverage that allow each employee to determine how much coverage is needed. The Bank pays 100% of the premium cost for the employee and all covered dependents for the Base Plan. Employees may purchase additional coverage and benefits for themselves and their dependents.

Life Insurance

The Citizens Bank provides each employee basic life insurance to help protect their families and other beneficiaries from a loss of income in the event of the employee’s death. All full-time employees are covered under two plans. The amount of coverage is based on the employee’s salary and levels of coverage vary based on the employee’s position. The Citizens Bank covers 100% of the cost.

Income Protection

The long-term disability benefit available to full-time employees provides a percentage of income if the employee becomes disabled through illness or accident for an extended period. The Citizens Bank covers 100% of the cost.

Long-Term Care

The Citizens Bank provides coverage for the payment of nursing home care and/or qualified home care for full-time employees that meet the service requirements. The basic coverage is covered at 100% by the Bank. Employees may purchase additional coverage for themselves or their spouse.

Supplemental Plans

The Citizens Bank offers employees voluntary supplemental benefits that are individually owned and conveniently payroll deducted. These plans include Cancer Insurance, Intensive Care Insurance, Accidental Insurance, Short-Term Disability Insurance, Specified Heath Insurance, Life Insurance and Flex Spending Accounts.

Cafeteria Plan

The Cafeteria Plan is a benefit established by the Bank to allow for medical, dental, vision and other premiums to be paid for with pre-tax dollars through payroll deductions. This plan is available to all full-time employees at no cost.

Employee Assistance Program

The Citizens Bank provides the REAL Employee Assistance Program to all employees, their spouses and dependents. This program provides help with stress, anxiety, depression, other emotional or behavior issues, as well as treatment for alcohol or substance abuse. The Citizens Bank covers 100% of the cost.

Vacation

The Bank provides paid vacations for all eligible employees for the purposes of rest and relaxation. Regular, full-time employees accrue up to two weeks of vacation based on their anniversary date of hire. Additional vacation accruals begin at service year 5 and build to a maximum of three weeks.

Sick Leave

Regular, full-time employees are eligible for sick leave following 90 days of employment. Additional sick leave accruals are based on years of service and build to a maximum of 30 days.

Retirement

The Citizens Bank provides a 401(k) Retirement Savings Plan to help employees accumulate financial resources for retirement. There is no age or service requirement to participate in the elective deferrals portion of the plan. The Bank makes a matching contribution equal to 50% of eligible elective deferrals up to the first 6% of employees’ contributions. The Bank may also provide a 3% Safe-Harbor contribution to the employee’s account. To be eligible to participate in the employer match or the safe harbor contributions, an employee must complete 12 months of service, have worked at least 1,000 hours and be 21 years of age.
